Medicare Advantage in Central Kentucky: A County-by-County Guide
Here is the single most important fact about Medicare Advantage that almost nobody tells you: your options are set by your county, not your state. A Lexington retiree chooses from 47 plans in 2026. Cross into Jessamine or Scott County and the menu changes; some plans disappear, premiums shift, and the "perfect plan" your Fayette County brother-in-law swears by may not even be sold where you live. This guide walks through the counties around Lexington one at a time: what shapes your choices there, which hospitals anchor your care, and how to compare plans for your exact address.
Why the county line matters so much
Insurance companies file each Medicare Advantage plan with CMS for a specific list of counties. That is the whole system. Premiums, drug coverage, dental extras, and, critically, hospital networks are approved county by county, which produces three real-world consequences:
- Plan menus reset at the county line. Fayette County's 47 plans are not Jessamine County's list, even though the two counties share a border and a hospital market.
- Advice that ignores your county is guessing. A national TV hotline quoting "plans in Kentucky" is averaging across 120 counties. More than 100 different Medicare Advantage plans are sold across the state in 2026, but no single county sees anything close to all of them.
- Moving counties can trigger a plan change. If you relocate and your plan is not sold in the new county, you get a Special Enrollment Period to pick a new one. Even a move from Lexington to Nicholasville can do it.
Original Medicare, by contrast, has no county lines and no networks. Pair it with a Medigap supplement and every hospital in this article accepts you. The county homework below is a Medicare Advantage exercise.
The 2026 backdrop: fewer plans, more surprises
2026 was the year carriers pulled back. Nationally, UnitedHealthcare exited 109 counties, Aetna closed roughly 90 plans, and the total number of Medicare Advantage plans fell about 10%. Extras like OTC allowances and flex cards were widely trimmed. In Central Kentucky, that showed up as discontinued plans and thinner benefits, and it landed on top of the local network breaks we track in the Lexington hospital network guide.
County by county: where you live shapes what you choose
The biggest menu, and the most network homework
47 Medicare Advantage plans in 2026, average premium about $9.33, roughly 21 plans at $0 premium. Lexington is also where all three major hospital systems live: UK HealthCare (Chandler, Good Samaritan, Markey Cancer Center), Baptist Health Lexington and Baptist Health Hamburg, and CHI Saint Joseph (Saint Joseph Hospital and Saint Joseph East). Big menu, big caveat: carrier network splits hit hardest here. UnitedHealthcare and WellCare plans currently keep you at UK but not Baptist; Humana, Aetna, and Anthem sit in-network at both. Check the current status before you fall in love with a premium.
Lexington's market, a different plan list
Jessamine County residents live in Lexington's medical orbit: Saint Joseph Jessamine on Keene Road is the county's only full-service 24/7 emergency room, and most hospital care happens up Nicholasville Road at the Lexington systems. That makes the Lexington network chess game matter just as much here, played from a different plan list, because Jessamine's county filing is its own. If your doctors span both counties (very common), verify each one against the specific plan.
A hometown hospital plus the I-75 corridor
Georgetown Community Hospital covers Scott County close to home, and the quick I-75 run puts UK, Baptist, and Saint Joseph within easy reach for specialists. The practical question for Georgetown retirees is whether a plan covers both the hometown hospital and the Lexington system you would want for something serious. Those are separate contracts, and plans do not all carry both.
A community hospital in transition
Winchester's community hospital, long known as Clark Regional Medical Center, recently rebranded as Centerpoint Health Winchester. Ownership and branding changes can come with payer contract changes, which makes this county a place to double-check network status rather than assume last year's answer. Lexington's systems sit a short drive west on I-64 for specialized care.
Baptist country, with a Lexington backstop
Baptist Health Richmond has served Madison and several neighboring counties for over a century, which gives the Baptist network question extra weight here: the carriers Baptist lists as out-of-network (UnitedHealthcare, WellCare, Essence as of 2026) are out at Richmond too, not just Lexington. UK HealthCare remains the regional referral center for complex care.
Live in Woodford, Bourbon, Franklin, or another Bluegrass county? The same rules apply: your county sets the menu, your nearest hospital system sets the network question, and the Kentucky county hub is growing to cover more of them. The Bluegrass Area Agency on Aging serves 17 Central Kentucky counties if you want free local counseling in the meantime.
How to compare plans for your county (not "Kentucky")
- Start with your ZIP code on Medicare.gov's Plan Finder. It filters to your actual county menu automatically. Never work from a statewide list or a mailer.
- Screen by your providers first, premium second. A $0-premium plan that puts your hospital out-of-network is not cheap. Verify your hospital and each doctor using the three-step check in the network status guide.
- Run your prescriptions through each finalist plan. The same drug can sit on different tiers in two plans sold in the same county.
- Re-check every fall. County menus, networks, and benefits reset every January 1. The Annual Enrollment Period (October 15 to December 7) is your yearly window to react.
Common questions
How many Medicare Advantage plans are available in Fayette County, KY in 2026?
Fayette County has 47 Medicare Advantage plans in 2026, with an average monthly premium of about $9.33, and roughly 21 plans with a $0 premium. Neighboring counties typically have fewer choices, which is why a plan recommendation that fits Lexington may not exist in Jessamine, Scott, Clark, or Madison County.
Why are my Medicare Advantage options different from my neighbor's in the next county?
Medicare Advantage plans are approved and sold county by county. An insurer files each plan for specific counties, so the list of plans, premiums, and benefits resets at every county line. Your ZIP code and county determine your menu, which is why comparing plans using your exact county is essential.
What hospitals serve Medicare patients in the counties around Lexington?
Fayette County: UK HealthCare (Chandler and Good Samaritan), Baptist Health Lexington, and CHI Saint Joseph (Saint Joseph Hospital and Saint Joseph East). Madison County: Baptist Health Richmond. Scott County: Georgetown Community Hospital. Clark County: Clark Regional Medical Center in Winchester (rebranded Centerpoint Health Winchester). Jessamine County: Saint Joseph Jessamine in Nicholasville for 24/7 emergency care, with most hospital care in Lexington. All accept Original Medicare; Medicare Advantage network status varies by plan.
What did the 2026 Medicare Advantage pullback change in Kentucky?
For 2026, carriers trimmed Medicare Advantage nationally: UnitedHealthcare exited 109 counties nationwide, Aetna closed roughly 90 plans, and total plan counts fell about 10%. In Central Kentucky that showed up as discontinued plans, changed benefits, and trimmed extras. If your plan was discontinued, you get a Special Enrollment Period and possibly guaranteed-issue rights to buy a Medigap plan, so a non-renewal notice deserves prompt attention, not the trash can.
Do I have to change plans if I move to a different Kentucky county?
If your current Medicare Advantage plan is not offered in your new county, you qualify for a Special Enrollment Period to pick a new plan when you move. Even moving one county over can change your plan menu, so report the move to your plan and compare your new county's options promptly. Original Medicare and Medigap travel with you anywhere in the country.
